Our patient portal was updated in December 2020. If you have not logged on since then, you will need to create a new account.
If you are interested in signing up for the patient portal, please call 801.773.4840, ext. 7500 or email us at patientportal@tannerclinic.com. Please provide your name, birthday and current email.
NOT ALL PROVIDERS currently use the patient portal. Please ask your provider if they use and and how you can best communicate with them
