Tanner Clinic owns your medical records as a legal document. To release all or portions of your record to third parties, including yourself, you must complete and sign an Authorization for Release of Protected Health Information form which is available below or at each reception desk. Tanner Clinic sends records directly to another physician at no charge. Tanner Clinic may charge a processing fee for any other release, payable before you pick up or send the records.
Please note, Westside Medical patients do not need to request for their medical records to be transferred to Tanner Clinic.
